Report a Planned Absence

This form is to be completed by a parent/guardian for an absence that is planned to occur at least a week in advance. Examples include family travel plans or a college visit.

Parents should email attendance@ycds.org to report daily absences for illness, etc.

Enter student first and last name.
Enter date(s) of planned absence.
If needed, provide other reason for absence.
If applicable, share the college to be visited.
